What are Mystic Meadows’ admissions office hours?

The admissions office is open Monday through Friday, 8:30am to 4:30pm, and the team is also available 24/7 to help with admissions outside of regular office hours. If you're admitted after hours, an admissions coordinator will schedule a time to meet with you as soon as possible to complete paperwork.

What should I bring when a loved one is admitted?

Please bring insurance cards, a COVID-19 vaccine card if applicable, a current medication list, and any other pertinent medical information. An admissions coordinator will meet with you after you're settled in your room to complete the necessary paperwork.

Do I need to bring my own medications?

No. Medications are provided through Mystic Meadows' contracted pharmacy. Upon admission, your hospital discharge medications are ordered and sent directly to the facility, so there's no need to supply your own unless a doctor instructs otherwise.

Who manages my medical care during my stay?

Every patient, whether here for short-term rehabilitation, a respite stay, long-term care, or hospice services, is assigned to a dedicated physician group. That physician and their clinical team oversee your care throughout your stay and are available to address daily medical needs.

Does Mystic Meadows accept Medicare and Medicaid?

Yes. Mystic Meadows accepts Medicare, Medicaid, private pay, and most major insurance plans, and is contracted with most long-term care insurance policies. Our business office can also guide you through the process of applying for Medicaid. Discharge time is typically between 9:00am and 11:00am on your designated discharge date. Our social services department works with you, your physician, and the rehabilitation team well ahead of that date to build a discharge plan, and for patients returning home, they help arrange equipment, home care services, and outpatient or home therapy.
